Inactive Coverage

If your Active coverage under this plan ends, there are circumstances under which you can receive a temporary extension of your health care coverage at group rates. This extension applies to you and your family.

As an employee, you have the right to extend your coverage if your coverage ends because:

• you leave employment with a contributing employer for reasons other than gross misconduct on your part, or
  
  
• you no longer meet the eligibility requirements
